Google Docs take 2

I created a word file on google docs for students who are new to the classroom.  Last time I used the spreadsheet format on google doc, where the students had to fill in the columns.  With the word file, I asked the students to answer the questions so I could learn some information about them.  This could be used in the beginning of the year or for students who have a high-mobility rate who move from school to school.  That happens frequently in my district.
